How we treat a patient with varicose veins at Dobrobut Medical Center

BE DIFFERENT

In this article, we will dwell on the nuances of treatment of varicose veins, especially the most difficult category of patients with advanced forms of the disease with the presence of complications. We will tell you about the features of our approaches to surgical treatment and how they differ from the generally accepted ones, what are our advantages and why you should choose our clinic for treatment.

Today there are the following requirements for surgical treatment of varicose veins:

  • Only an individual approach
  • Quality topical diagnostics
  • Taking into account the wishes and expectations of patients
  • Ambulatory performance
  • Active use of visualization methods at all stages
  • Minimizing surgical trauma
  • Painlessness
  • Maximum security
  • Aesthetics and high cosmetic effect
  • Quick activation and subsequent rehabilitation
  • Efficient use of material resources
  • Reasonable price

Taking into account all these requirements, of course, the most difficult task is the treatment of patients with massive and widespread lesions of the subcutaneous veins, with pronounced violations of venous blood circulation in the lower extremities and the presence of complications - trophic skin changes and ulcers. The difficulty lies in the fact that in such cases both standard and modern methods of surgical intervention can be carried out in a very limited way.

Firstly, at such stages of the disease, a large number of technical difficulties arise, which sharply limit the possibility of using modern technologies due to the severity of the lesion, the size of the veins, their significant deformation, transformations, etc. d.

Secondly, the presence of trophic skin changes makes it impossible to use the most effective method for such an injury - surgical removal - due to the 100% probability of developing complications in the form of infections and non-healing wounds with the formation of new trophic ulcers.

Thirdly, as is known, in our country the absolute majority of patients with varicose veins have an advanced or complicated form. This condition is further aggravated by the fact that even narrow specialists, vascular surgeons, realizing the difficulty of treating such patients, usually refuse them surgical assistance, not wanting to take responsibility for possible adverse results of operations. In addition, extremely advanced forms of varicose veins occur most often in elderly patients. The presence of accompanying chronic diseases, sometimes severe enough, at a certain age makes it impossible to use standard methods of anesthesia, which are required for surgical treatment.

Started forms of the disease with the presence of complications

The disease brought to such a stage is a problem not only for the patient, but also for the specialist who carries out the treatment. The massiveness of the lesion and the significant size of the veins that need to be removed make it extremely difficult to perform minimally invasive surgery. There are clear and standard requirements, which are used all over the world, for the diameter of the main (main) veins, which can be treated with modern methods - such as laser or radiofrequency ablation, catheter scleroobliteration. When using these methods on significantly dilated veins, their effectiveness is sharply reduced and the risk of developing complications, as well as early recurrence, increases. The time of the operation increases significantly, its performance in outpatient conditions is called into question, since the duration of the outpatient operation should be up to three hours. Massively varicose veins require incisions, albeit minimal, but such that they already require sutures. In this case, the postoperative period will not be so comfortable and painless. In such cases, the likelihood of complications, large hematomas, hemorrhages, swellings, which are common in standard operations for varicose veins, also increases. As a result, all this leads to a significant increase in recovery and rehabilitation periods after surgery.

In the presence of trophic skin changes on the lower leg, everything is complicated by the impossibility of performing punctures and incisions in this area due to the high probability of ulcers and infection. In this case, in order to minimize the trauma of the operation and reduce the risk of complications, it is possible to divide the surgical treatment into certain stages, as well as to combine various treatment methods.

The most difficult situation is the presence of trophic ulcers, which occurs already in the terminal stage of the disease. Changes in venous blood circulation in this stage are maximally expressed, severe in nature, the lesion usually affects not only superficial, but also deep veins, and sometimes affects the arterial bed. At the same time, the diameters of subcutaneous veins most often reach those values at which laser surgery is contraindicated, and the size of varicose veins dictates the need to remove them only through incisions. In addition, the presence of trophic ulcers always implies an infectious lesion of the skin, which limits the possibility of performing any surgical manipulations in an outpatient setting.

But even such a situation is not hopeless for a trained specialist. Approaches to the treatment of such patients are based on a long, gradual and precisely calculated combination of various methods. Such treatment includes compression, drug local therapy, and then a combination of surgical techniques, individually selected for each patient and based on accurate ultrasound diagnosis. In any case, with a professional approach, it is always possible to at least ease the course of the disease, at the most - to achieve its permanent regression, heal ulcers with a guarantee of their non-return, and minimize the manifestations of the disease.

Conditions for the treatment of varicose veins

We consider it necessary to note that the ""Dobrobut"" clinic has all the necessary conditions that meet international standards for the successful treatment of patients with varicose veins at absolutely any stage. We have a comfortable hospital with round-the-clock monitoring, intensive care wards, qualified and trained staff, and a high-level anesthesiology department. This allows us to be very flexible in the choice of tactics and method of treatment, to apply all currently known achievements of phlebology, to perform the entire range of surgical operations and manipulations. However, we still prefer modern methods of surgical treatment of varicose veins, which are used in outpatient settings, that is, without hospitalization of the patient.

We work only with the best equipment, we use modern ultrasound devices for diagnosis and control of manipulations carried out during surgery. We use a modern laser device of the German company ""Biolitec"" with the most efficient two-ring light guides, which are recognized as the best in the world.

In addition, our qualifications and constant pursuit of improvement allow us to apply the most effective methods and technologies, as well as the rich experience of other specialists from around the world, from whom we had the honor to learn.

A rich and diverse arsenal of knowledge and practical skills allowed us to develop our own methods of treatment of advanced forms of the disease with severe complications, based on an optimal combination of all modern technologies and approaches, including non-traditional methods of pain relief, regardless of age and the presence of concomitant diseases.

Our main principle is effective treatment ""at one time""

As you know, most specialists around the world, including in Ukraine, treat patients with advanced forms of varicose veins in several stages. At the same time, they are guided by the principle of ""temporary improvement"" of the condition and explain this approach by the fact that varicose veins are a chronic disease that requires periodic treatment. Indeed, as a rule, patients note a significant improvement in their condition, disappearance of complaints and reduction of varicose veins after stage operations. But this result is almost always temporary, the manifestations of the disease return over time and force patients to actively seek treatment again, which reduces daily activity, distracts from work, changes the routine of life and causes new costs. This approach can be considered acceptable in the conditions of Western insurance medicine, but, according to our deep conviction, it is categorically not suitable for patients of our country with limited opportunities for treatment. It should be noted that there are also such clinical situations when it is still necessary to carry out treatment in several stages, but this is rare. In our subjective opinion, if there is an opportunity to cure the patient at once, despite various technical difficulties, it must be done in this way. After all, the task of a professional is to solve complex problems and find the optimal solution.

Regarding the acceptability of the price

Of course, modern treatment technologies using innovative methods and expensive equipment cannot be cheap. If we already live in the conditions of a market economy, then manufacturers of medical equipment, of course, try to get the maximum benefit from the price of consumables, which mainly consists of the relatively high cost of modern surgery. However, this is relative.

Firstly, it is always necessary to take into account the obvious advantages of modern surgery - no need to stay in a hospital, a painless postoperative period, no restrictions on physical activity, which allows you to lead a normal lifestyle. In addition, it does not violate the usual order of family life, does not limit the volume of household chores. In this case, the patient can immediately begin to ""recover"" his financial expenses, returning to work the next day after the operation. For comparison: for a normal operation, the patient stays in the hospital for at least 3-4 days, the operated leg hurts, it is impossible to adequately load the legs, the patient will be able to return to work only after a few weeks. The ability to quickly return to an active lifestyle, sports, etc., is also of great importance for a modern young person. - she will be able to do it only a month after the modern operation, and after the standard one - after 2-3 months. The same period is necessary for elastic compression of the operated leg, which also causes a lot of inconvenience and discomfort.

Secondly, one should always treat one's health adequately and evaluate it and the offered medical services from the point of view of comparison with usual things, the cost of products, etc. It is also worth remembering the importance and value of one's health, its irreparability, as well as the fact that working capacity, that is, the ability to provide for oneself and one's family, is directly related to well-being. Therefore, the efforts of Western medicine are aimed more at prevention than at treatment, which we should get used to. Therefore, it is precisely in the West that modern methods of treatment are being developed, which allow to quickly and effectively treat various diseases. All people should understand that treating an advanced disease is even more expensive, so you need to seek help as early as possible.

Third, we all need to wean ourselves off the idea of free medicine as soon as possible. Everything in this life has its price and no service can be charitable. All over the world, medicine is a service industry, and it is treated accordingly. And only here it is perceived as help - it is a relic of the Soviet past. Medical professions require long-term training, long-term accumulation of experience, continuous self-improvement of skills, as well as enormous responsibility. Throughout history, medicine was exclusively a craft, requiring the necessary knowledge and skills, which, of course, always paid. It is necessary to rebuild one's perception of medicine and one's own health as early as possible. Of course, in the field of services, there are always enough different options, which will differ both in terms of quality and price, and from this point of view, the cost of modern, higher quality and more convenient treatment methods in comparison with standard ones also becomes clear.

Fourthly, in the evaluation of the offered medical services, it is still necessary to use mathematics. For example, if you have been ill for 5 years, then if you divide the cost of the operation by the number of days of your illness (1825 days), it will be pennies that can be spent per day of effective treatment. And you can estimate the future years by comparing the quality of your life with the disease and a ""healthy"" life, taking into account the gradual deterioration of well-being.

Fifthly, which concerns treatment specifically in our conditions. All surgical interventions offered by us are of an exclusively radical nature, that is, we always try to perform an operation aimed at removing all affected veins at this moment, which has the maximum guarantee of cure from the disease, provides the patient with confidence that no treatment will be needed in the future. This is a non-standard attitude to varicose veins in modern phlebology, when the treatment is put on the flow, and the patient is tied to the clinic for the constant performance of some procedures or manipulations. Of course, in this variant of the offered services, the patient's one-time costs for treatment will be much lower, but in the end, the treatment will cost much more than a one-time operation. This is a medical business, however, the same as any other. We try to help the patient so that he is considered completely healthy after our procedures. In addition, we never leave our patients, we observe them on an outpatient basis for a long time, ensuring control and timely treatment if necessary, we never refuse additional examinations, examinations, etc. d. We always meet the wishes of our patients, because the main thing for us will not be business, but people's health.

Principles of work of MC ""Dobrobut""

Confidence in one's own abilities, many years of experience, a wide range of knowledge, a large arsenal of practical skills, a variety of proposed treatment options, frankness with the patient in discussing his condition, involvement of the patient himself in choosing the optimal treatment option - these are, in our opinion, the main features a high-level professional.

We are deeply convinced that the patient's comprehensive understanding of his disease, his awareness of the lack of alternative treatment and the need to conduct it in a timely manner, as well as the patient's involvement in the process of choosing tactics, are a necessary condition for communication between a professional doctor and a patient. We always take the patient's wishes into account. This approach creates an aura of trust, compliance (agreement, commitment to treatment) ensures a positive attitude of the patient and is a guarantee of a quick recovery. Our patient is always aware of how they plan to treat him, what will happen at all stages, and we do everything possible not to disappoint him.

Our rich professional experience testifies to the great variability of the forms of varicose disease, a significant number of non-standard situations and features of leg vein damage. We never work on a routine basis and never create a ""conveyor belt"". We individually select the optimal treatment tactics for each patient. This approach is ensured by accurate and timely ultrasound diagnostics, which we actively use before and after surgery.

Regardless of the form and stage of the disease, we base our work on the principle of ambulatory care. The ability to perform operations and manipulations without hospitalization is the best option for the absolute majority of patients. For treatment, we always choose the least traumatic method, which allows the patient to return to his usual routine immediately after the operation. In addition, all our interventions are distinguished by a high degree of cosmetic effect and aesthetics.

An important fact is also that we never leave patients unattended. All our patients are monitored by telephone in the early postoperative period, and doctors are always available to the patient. Patients are actively monitored after operations, a dispensary supervision system has been developed with mandatory periodic examinations and control ultrasound. We monitor the quality of life of our patients before and after surgery, which allows us to adjust approaches to treatment, improve the technique of operations and manipulations to achieve the maximum possible effect for the benefit of patients.

We constantly develop professionally, never stand still, try to master new effective treatment methods, regularly participate in special medical events, raising our level.

We actively participate in the scientific life of the medical community and regularly publish data about our experience and the results of our work in Ukraine and abroad, defending our approaches to treatment and views on modern treatment options for varicose veins.

We hope that the given explanations will help you understand the motto we have chosen. We were prompted by the slogan of the APPLE company ""Think Different"" - think differently.

“Be Different” – be different. Still, there is more action and activity in this phrase, which fully corresponds to both our specialty - surgery, and our art of manual therapy (this is how the word ""surgery"" is translated), and the most effective method of treating varicose veins. We understand it this way: we will not be like other specialists - in an effort to help absolutely every patient, regardless of the difficulties, taking the patient's health as a basis, and not other principles. And you will be different, different from other patients in your desire for timely treatment and speedy recovery.
