Advantages of minimally invasive cardiac surgery
Ukrainian cardiac surgery has taken a step forward - important and complex heart operations are performed minimally invasively, that is, without traditional large incisions and ugly scars after them. According to the practicing doctors themselves, 15 years ago they did not imagine such operations and did not believe in the success of their implementation.
Gist
Patients often ask: Is minimally invasive cardiac surgery promising? What is it anyway?. The essence of such operations is that an intercostal incision of no more than 5-10 cm in length is made as a surgical access. At the same time, they perform not simple interventions, but full-fledged operations on the heart.
Some modern realities should be taken into account:
- such operations can be performed only in those patients who have a single cardiac problem - for example, a violation of the aortic valves, which requires their prosthetics. In the presence of combined pathology, classical operations are practiced with conventional access - an incision of up to 25 cm;
- only a number of Ukrainian medical institutions dared to switch to mini-invasive methods. Such a decision depends on the technical equipment of the clinic (in particular, its operating room), as well as on the experience and psychological adjustment of the doctors.
The "Dobrobut" medical network has such equipment, and its cardiac surgeons have the necessary practical skills in performing minimally invasive operations. Currently, in the clinic, about 2/3 of patients with cardiac problems undergo minimally invasive surgery.
The clinic doctors propose to solve the problem of isolated operations in the following way - to carry out so-called hybrid operations (simultaneous use of endovascular and surgical methods during the same surgical intervention). According to forecasts, this will be possible in a few years.
Advantages
The main advantages of minimally invasive cardiac surgery:
- such operations are less traumatic than usual ones, so patients suffer much less pain;
- the risk of developing traditional postoperative complications (for example, congestive pneumonia) is reduced to a minimum;
- after a minimally invasive intervention for complex heart pathologies, patients recover quickly, return to a normal rhythm of life already on the third day, and are discharged from the hospital on the fifth.
List of minimally invasive operations
How is the heart treated minimally invasively and what operations are performed? The following surgical interventions are performed:
- multivessel coronary bypass. It is the most effective method of treating CHD (ischemic heart disease), which allows you to fully restore blood flow in the heart muscle even with critical damage to the coronary vessels and in the presence of a history of myocardial infarction. Cardiac surgeons of the "Dobrobut" clinic developed the technique of such an operation;
- aortic valve plastic surgery and prosthetics. At the same time, synthetic and biological prostheses are used;
- plasty and prosthesis of the mitral valve - performed in case of its insufficiency. There are described cases when, after such an operation, patients passed a medical examination, were allowed to work, and they returned to the field that requires significant (and not very low) physical training;
- plasty and prosthetics of the tricuspid valve. Such operations are performed for acquired and congenital valve defects - this can also be attributed to the advantages of minimally invasive cardiac surgery.
In addition to multivessel coronary bypass surgery and operations on heart valves, the minimally invasive method is practiced in the surgical treatment of other pathologies:
- disease of the ascending aorta;
- a neoplasm of the heart;
- congenital defects (developmental anomalies) of the heart - atrial septal defect, incomplete anomalous connection of the pulmonary veins, and malformation of the tricuspid valve.
