Coagulogram interpretation in adults: main indicators

Coagulogram - what kind of analysis is it? Coagulogram (hemostasiogram) is a complex blood test necessary to study indicators of the hemostasis system. The analysis is prescribed during pregnancy, with pathologies of the liver, blood vessels, immune system, before surgical operations. What does the expanded coagulogram show? Thanks to the hemostasiogram, it is possible to determine the prothrombin index (PTI), international normalized ratio (INR), thrombin time (TR), fibrinogen, lupus anticoagulant, D-dimer, antithrombin III, activated partial thromboplastin time (APT) and other indicators.

What is included in the extended coagulogram:

  • EOM;
  • PTI;
  • fibrinogen;
  • D-dimer;
  • АХТЧ;
  • PM;
  • antithrombin III.

When it is necessary to take a blood test for a coagulogram

The results of the hemostasiogram make it possible to identify various disorders in the blood coagulation system and prescribe timely treatment for the prevention of bleeding and the formation of blood clots. The test must be taken before surgery and during pregnancy to assess the risk of bleeding or blood clots.

It is also necessary to take a blood test for a coagulogram if:

  • repeated miscarriages;
  • liver function disorders;
  • varicose veins;
  • heart attacks;
  • strokes;
  • anemia;
  • profuse menstrual bleeding;
  • long-term use of glucocorticosteroids or oral contraceptives.

The coagulogram includes indicators reflecting various functions of the blood coagulation system. Usually only some of them need to be defined. The doctor decides which indicators of the hemostasiogram need to be determined for the patient.

Coagulogram parameters

Coagulogram interpretation in adults:

  • AChT – the time during which the blood is able to clot (the duration of clot formation). Any deviation from the norm indicates disseminated intravascular coagulation. In the table with norms of coagulogram, indicators of AChT of 23-37 seconds are indicated;
  • fibrinogen is a specific protein produced in the liver. Fibrin threads are formed from it, which participate in the process of blood clotting and thrombus formation. The norm of fibrinogen in the blood is 2-4 g/l (during pregnancy – up to 6.5 g/l);
  • TH (thrombin time) is the period during which fibrinogen turns into fibrin threads. Normal values are 11-18 seconds;
  • prothrombin index (PTI) is the ratio of the patient's blood clotting time to the accepted standard. The norm is 97-100% (during pregnancy - up to 150%);
  • Plasma recalcification time (PRT) reflects the rate of fibrin clot formation when calcium is added to blood plasma. The indicator provides information on the activity of the entire blood coagulation system. The norm is 60-120 seconds;
  • antithrombin III is a protein that ensures the functioning of the blood clotting system. Normal values are 70-115%;
  • D-dimer is a protein fragment that can be detected in the blood after the blood clot dissolves. The concentration of D-dimer is affected by the size of the thrombus, the use of anticoagulants, pregnancy and the patient's age. Variants of the norm - 33-726 ng/ml.

Hemostasiogram indicators depend on a number of factors (patient's age, chronic diseases, drug therapy) and may deviate from the norm both in the direction of decrease and in the direction of increase. Coagulogram indicators during pregnancy also change. This is due to hormonal changes, an increase in the volume of circulating blood and other factors. The indicators are evaluated based on the compliance of the period (trimester of pregnancy) with the reference values.

Preparation for hemostasiogram, analysis results

The analysis is given on an empty stomach. The last meal is no later than 8 hours before the examination. It is necessary to inform the doctor in advance about taking medicines. Blood is taken from a vein. The price of a coagulogram depends on the level of the clinic and the number of indicators for which the analysis will be performed. The more studies are assigned, the higher the cost of a comprehensive analysis.

How long does a coagulogram take? Execution time - 24-48 hours (depends on the form of the hemostasiogram). The deadlines are also affected by the workload of doctors and laboratory technicians.
